At Check-In
Please arrive 20 minutes prior to your first appointment
Please bring a valid photo ID and your Insurance Card
Please bring a form of payment (cash, credit, debit, check) for Co-pays, deductible or self-pay visit fees
Please bring all medications you are taking including prescriptions, over-the-counter drugs, herbal products and vitamins
Bring a list of allergies – especially any concerning medicine or food
At Check-Out
We will provide you with an after-visit summary
We will provide you with any printed orders or any referral information you may need
We will schedule your diagnostic and follow up appointments
We will collect any visit fees that are due
We will send any prescriptions that are needed to your pharmacy of choice

Please note: Children under age 18 must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement authorizing treatment, signed by the parent or guardian.
