ChartSpan​ Care Management

MedFirst partners with Chartspan to offer care management services to our Medicare patients. The goal of a care management program is to assist your Med First Provider in coordinating your healthcare, ensure that you have access day or night to a nurse line for any questions or concerns, and ensure that you have the resources and support to meet your healthcare goals and live a healthier life. There are many advantages to the care management program, including:

• A personalized care plan
• A nurse line, available 24/7 for health questions or concerns
• Help with medication costs and refills
• Transportation services to get to appointments if needed
• A team that cares about your health

All of these benefits help keep you meet your healthcare goals. A healthier you means a higher quality of life and lower healthcare costs.

So, how do you sign up?

If you are a Medicare insurance participant, you may be eligible. Just let your Med First provider know you are interested, or call Chartspan at (855) 975- 2253 to enroll or learn more. A monthly copay and deductibles apply.
